چکیده
Limitation of exercise capacity is a hallmark of chronic congestive heart failure (HF), varying directly with the severity of the disease. Muscular fatigue and dyspnea on exertion are the most common symptoms of exercise intolerance; however, their origin is still not fully explained. Heart failure is a multifaceted clinical syndrome characterized by the inability of the heart to generate cardiac output at levels capable of meeting the metabolic needs of the organism associated with metabolic and inflammatory disorders and neurohormonal activation. Several studies have shown that exercise intolerance in patients with chronic HF results from a complex interaction between the pulmonary, cardiovascular, skeletal muscle, hematopoietic, and neurohumoral systems. Mechanisms that could limit exercise capacity include the heart’s inability to sufficiently raise cardiac output during effort, chronic elevated systemic vascular resistance, abnormal peripheral vasodilatatory capacity, and abnormal distribution of blood flow with subsequent tissue hypoperfusion, metabolic dysfunction in skeletal muscles, and impaired pulmonary ventilation capacity, all of which are characteristics of HF. The measurement of respiratory gas exchange during a maximal exercise test, used to obtain peak oxygen consumption, is established as an objective and reproducible method for assessing exercise capacity and prognosis in patients with HF. The 6-min walk test (6MWT) is a simple test that does not require expensive equipment or advanced training for technicians. The patient is asked to walk the longest distance possible on a walking course (eg, a corridor), preferably at least 30 m long, in a set interval of 6 min. Furthermore, due to its wide availability and the safety and ease of implementation, it is being used increasingly as an alternative to cardiopulmonary exercise testing to evaluate the functional capacity of patients with HF. The 6MWT is considered a submaximal exercise that mimics everyday activities and is generally well tolerated by patients. The distance walked during this test is a strong and independent predictor of morbidity and mortality in patients with left ventricular (LV) dysfunction.

اثر PercutaneousCoronary Intervention Elective بر بهبودی نارسایی ایسکمیک میترال
Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...

ABSTRACT: A consecutive Series of 522 patients (mean age 40.36 years ,female 70.9% ,male 29.1% ) with rheumatic valvular heart disease were studied during a period of seven years ,with particular reference to understanding the prevalence and risk factors of atrial fibrillation (AF) and systemic arterial thromboembolism (SAT). AF was observerd in 165 (31.6%) patients .The frequency of AF were ...
متن کامل[Echocardiographic predictors of exercise capacity in patients with heart failure and systolic dysfunction: role of mitral regurgitation].
INTRODUCTION AND OBJECTIVES Patients with heart failure and similar left ventricular systolic dysfunction have differing exercise capacity. The aim of this study was to identify echocardiographic predictors of exercise capacity in patients with heart failure and systolic dysfunction. METHODS We included 150 patients with class II (70%) or III (30%) heart failure with left ventricular ejection...
متن کاملCorrelation between Hyponatremia and High Risk Clinical and Echocardiographic Features in Patients with Acute Heat Failure
Background and purpose: Heart failure (HF) is characterized by decreased ability of the heart to provide sufficient blood flow or fill with the blood. Hyponatremia is the most commonly seen electrolyte abnormality in patients with heart failure that is associated with increased morbidity and mortality. The aim of this study was to assess the correlation between hyponatremia and high risk clinic...
متن کاملPredictors of long-term outcomes in patients with significant myxomatous mitral regurgitation undergoing exercise echocardiography.
BACKGROUND Significant myxomatous mitral regurgitation leads to progressive left ventricular (LV) decline, resulting in congestive heart failure and death. Such patients benefit from mitral valve surgery. Exercise echocardiography aids in risk stratification and helps decide surgical timing. We sought to assess predictors of outcomes in such patients undergoing exercise echocardiography. METH...
متن کاملThe impact of baseline left ventricular size and mitral regurgitation on reverse left ventricular remodelling in response to carvedilol: size doesn't matter.
b Blockers have been shown to reverse left ventricular remodelling (LVR) and to reduce the severity of functional mitral regurgitation (FMR) in patients with chronic systolic heart failure. It is unclear, however, to what extent the beneficial effects of b blockers on LVR or FMR are influenced by the severity of left ventricular dilatation or FMR at the time of commencement of b blocker treatme...
